General
Admission Criteria
This
agency complies with Title XX of the Civil Rights Act of 1964. This
means no individual will be excluded from Big Lakes' services on the
basis of race, color, national origin, sex, religion, age or disability.
Priority will be given to those who are from or who originate from Big
Lakes' service area (Riley, Geary, Clay and Pottawatomie counties in
Kansas).
This agency is partially funded on a contractual basis with the Kansas
Department of Social and Rehabilitation Services. Individuals must meet
the following eligibility guidelines as defined in Kansas law;
Mental Retardation
means substantial limitations in present functioning that is manifested
during the period from birth to age 18 years and is characterized
by significantly sub-average intellectual functioning existing concurrently
with deficits in adaptive behavior including related skills in two
of more of the following applicable adaptive skill areas:
1.
Communication
2. Self-care
3. Home living
4. Social skills
5. Community use
6. Self-direction
7. Health and safety
8. Functional academics
9. Leisure
10. Work
A "developmental
disability" is defined as a severe, chronic disability
of a person that: (a) is attributable to a mental or physical impairment
or combination of mental and physical impairments; (b) is manifested
before the person attains 22 years of age; (c) is likely to continue
indefinitely; and, (d) results in substantial functional limitations
in three (3) or more of the following areas of major life activities:
i. self-care,
ii. receptive and/or expressive language,
iii. learning,
iv. mobility,
v. self-direction,
vi. capacity for independent living,
vii. economic self-sufficiency,
viii. reflects the personās need for a combination and sequence
of special, interdisciplinary, or generic care, treatment, or
other services which are of a lifelong or extended duration and
are individually planned and coordinated.
